He clearly manifests a very strong narcissistic trait in his personality, he craves admiration and respect, but also knew that what he did would make him the pariah of Norway, Europe, and indeed the world. Using video games to prepare for a killing spree also manifests a naive, childish, streak. There is no evidence to suggest that the "Knights Templar", the organisation which he claims to belong to even exists, so it could probably just be a product of his diseased imagination, and an illusion cultivated to give the impression that he is not just a crazy loner mass murderer, but belongs to a very secretive, dedicated, esoteric group of far-right militant extremists.
But here's the irony: he claims to oppose National Socialism ie. Nazism, instead viewing those he gunned down as the Nazis, and the multiculturalist agenda which they promoted as being real Nazism. He uses the term "Cultural Conservative" to describe himself and the alleged movement he belongs to, and rejects the neo-Nazi label. He doesn't fear death, in fact he has stated that he welcomes it; viewing it as much more suitable and noble punishment to 21 years in prison, which he views as "a pathetic punishment". So on an intellectual level he is aware of the severity of his crimes, but seems to be lacking on the emotional level.
I think what the Norwegian courts have in front of them is someone who is unquestionably politically and ideologically motivated, and who is also either a bonafide psychopath, someone very high up the psychopathy scale with corresponding lack of conscience, guilt, and remorse, or someone who suffers for Narcissistic Personality Disorder with delusions of grandeur. Or he may just be a plain old paranoid schizophrenic.
He clearly put a lot of thought, planning, and preparation into his act of terror, and in strictly terrorist terms, his operation has been a success. He never expected to survive the day of his dual attack, so being in court and having the opportunity to air his "Cultural Conservative" views is simply an added bonus. He has asked the courts to acquit him or execute him. They're not going to do the former, and if they do execute him instead of sending him to prison, he wins, as they have made a martyr out of him, which is what he wants. The only way he loses in this is if they find him insane and send him to a psychiatric facility.
